Bottom line

The reference-standard ablative CO2 — the highest-energy aesthetic CO2 on the market and the only one with SCAAR FX for deep traumatic/burn scar work, modernized as of its 2023 US clearance. Also the most expensive device in this category and the most physician-dependent to operate — a medspa without a resurfacing-trained MD/DO on site has no business owning one. ROI is excellent where clinical talent exists: full-face tickets of $3,000-6,000 amortize it quickly. Buy it for surgeon- or dermatologist-led practices; buy a CO2RE-class device for lighter-duty fractional CO2.

How it works

The science

A 10,600nm CO2 laser whose energy is absorbed by tissue water, vaporizing controlled columns or layers of skin and coagulating surrounding tissue for aggressive collagen remodeling. Modes: ActiveFX (superficial fractional — tone/pigment/fine lines), DeepFX (deep fractional microcolumns for wrinkles and acne scars), TotalFX (both), SCAAR FX (very high-energy pulses to ~4mm for hypertrophic, burn, traumatic scars). Defining trait: very high pulse energy in short pulses — more ablation with less unwanted heat spread than lower-powered CO2s. Also serves surgical and gyn uses via dedicated delivery devices.

UltraPulse is high-peak-power short-pulse CO2 delivery — enough energy per pulse to vaporize cleanly before heat diffuses. SCAAR FX (Synergistic Coagulation and Ablation for Advanced Resurfacing) is ultra-high-energy fractional pulses penetrating several times deeper than standard fractional CO2 — thick fibrotic scars no other aesthetic CO2 reaches.

Best-fit practice

Dermatology, facial plastics, oculoplastics practices with an MD performing resurfacing, and scar-focused centers needing SCAAR FX depth. Patients skew Fitzpatrick I-III, 45-70, accepting 5-10 days of downtime for one-and-done correction. A volume medspa without physician operators cannot legally or safely monetize this machine in most states.

Compliance

Medical director & delegation

Class II but functionally physician-tier: fully ablative CO2 resurfacing is restricted to physicians (or tightly supervised NP/PA at most) in many states, and sedation considerations for full-face work raise the bar further. An absentee medical director model is inappropriate here.

Contraindications & screening

Isotretinoin within 6-12 months, keloid/hypertrophic scarring history, active infection (mandatory HSV prophylaxis for perioral work), Fitzpatrick IV-VI (PIH/hypopigmentation risk), prior lower-lid blepharoplasty for periorbital treatment, smoking and poor wound-healing states.

1-year standard; extended $10,000-18,000/year and worth it given tube and optics costs. This platform genuinely needs OEM-level support — third-party options limited for the Alpha generation.
