Bottom line

The niche play: a 1470nm laser-lipolysis platform whose real selling point is closed-loop fat harvesting and transfer — lipo, graft processing, and reinjection in one sterile circuit. For a natural-fat-augmentation practice (facial grafting, hands, small-volume body transfer) it's the most purpose-built system on the market. But it's a single-purpose story from a company with a thin US surgical footprint, the used market is shallow, and most surgeons get comparable grafting results from VASER or syringe systems at lower cost.

Overview

Laser harvesting for fat grafting

How it works

The science

Based on Alma's LipoLife platform — a 1470nm diode laser fires radially from a fiber inside the aspiration cannula, simultaneously loosening fat and coagulating tissue while low-vacuum suction draws fat into a closed sterile collection system. The wavelength targets water and fat with shallow penetration, so energy exposure of harvested fat is brief — Alma's core claim is high adipocyte viability (~95% in company-cited work). Harvested fat is filtered and transferred without leaving the closed circuit, cutting contamination risk and OR time, then reinjected for facial, hand, breast, or buttock volume.

Patented tech

The LipoLife radial-emission fiber-in-cannula design plus closed-loop harvesting/transfer circuit; companion LipoFlow automates wash/filter/transfer.

Origin

Alma Lasers Ltd., Caesarea, Israel (Sisram/Fosun Pharma); US distribution via Alma Inc., Buffalo Grove, Illinois.

Treatment economics

What practices charge

Combined lipo-plus-fat-transfer: ~$6,000-12,000 facial fat transfer with donor-site lipo; $10,000-20,000+ body transfer cases; standalone laser lipo of a small area $3,500-7,000.

Consumables & recurring cost

Sterile closed-loop kits (cannula/fiber assemblies, canisters, transfer sets) ~$300-700 per case through Alma — verify; annual spend scales with case volume.

Ideal for

Best-fit practice

Facial plastic, plastic, and cosmetic surgeons whose brand is natural volume restoration and regenerative aesthetics — surgeon-owned, not a delegated medspa service. Patients: 40s-60s facial volume-loss patients rejecting filler maintenance.

Compliance

Medical director & delegation

Laser lipolysis and autologous fat grafting are surgery — physician-performed; fat handling also implicates FDA HCT/P same-surgical-procedure requirements, so keep everything in one session.

Contraindications & screening

Pregnancy, coagulopathy, active infection, significant comorbidities, unrealistic volume expectations; caution in smokers (graft take) and large-volume debulking cases.

Ownership

Warranty & service

~12-month Alma warranty; Alma's US service is solid on energy devices but the surgical install base is small — confirm local field coverage before signing.

Resale & used market

Shallow, illiquid — few units in circulation, listings sit; expect steep depreciation (~$30k-60k used) and plan to hold to end of life.
